Registering With Us


We are currently accepting new patients that reside within our catchment area.


How to register


We can only accept new patients who live in our practice area so please check with the Receptionists that you are living in our area. If you are out of our catchment area please see guidance below. The practice serves the villages of Malvern Link, Newland, Leigh Sinton, Bastonford, Colletts Green, parts of Powick, Deblins Green and parts of Callow End.


You can register by completing a registration form which can be obtained from the practice or complete and forward your medical card. When you register, we will ask you to complete a new patient questionnaire. This will give our health care practitioners a brief history of your medical conditions until your medical records arrive from your previous practice.


If you are on regular medication you may have to see the doctor before your next prescription is due, so please check with the Reception Staff. This does not preclude your making an appointment for a new patient check if you wish, particuarly the over 75s, or those who have not seen their doctor within the last 3 years, and those patients who have substantial caring responsibilities for a friend or relative.


Instructions on what to do with the registration form


If you want to join the practice please complete a registration form which can be picked up from reception together with a New Patient Questionniare for all members of your family.


Please bring the completed forms along to reception with your medical card if you have it. The information on the medical card will speed up the process of getting your records from your previous practice.


Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
