We are currently accepting new patients

 

How to register

 

We can only accept new patients who live in our practice area so please check with the Receptionists that you are living in our area. If you are out of our catchment area please see guidance below. The practice serves the villages of Malvern Link, Newland, Leigh Sinton, Bastonford, Colletts Green, Powick, Deblins Green and parts of Callow End.

 

You can register by completing a registration form which can be obtained from the practice or complete and forward your medical card. When you register, we will ask you to complete a new patient questionnaire. This will give our health care practitioners a brief history of your medical conditions until your medical records arrive from your previous practice.

 

If you are on regular medication you may have to see the doctor before your next prescription is due, so please check with the Reception Staff. This does not preclude your making an appointment for a new patient check if you wish, particuarly the over 75s, or those who have not seen their doctor within the last 3 years, and those patients who have substantial caring responsibilities for a friend or relative.

 

Instructions on what to do with the registration form

 

If you want to join the practice please complete a registration form which can be picked up from reception together with a New Patient Questionniare for all members of your family.

 

Please bring the completed forms along to reception with your medical card if you have it. The information on the medical card will speed up the process of getting your records from your previous practice.

 

Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

 

Out of area registrations

 

New arrangements introduced from January 2015 give patients greater choice when choosing a GP Practice. Patients may approach any practice, even if they live outside the practice area, to see if they will be accepted on to the practice list.

 

The new arrangements mean we now have the option to register patients who live outside our practice area but withoutthe option to provide home visits. Out of area registrations is a voluntary arrangement for practices.

 

We will only register out of area patients if it is clinically appropriate and practical in your individual case.  For example, if you have complex medical needs and would require home visits, it would not be appropriate to register you as an out of area patient.

 

If you would like register as an out of area patient, you will need to write a letter (addressed to the Reception Manager) which includes the following information for each member of your household who would like to register with us:

 

  • Name, address and date of birth
  • Name, address and telephone number of your existing practice
  • Confirmation that you are happy for us to contact your existing GP for further information about your existing conditions should we require this
  • Details of any long term conditions or illnesses that you may have
  • Brief reasons why you would like to register with us

 

Your request will then be considered by a GP. They will base their decision on whether to accept you as an out of area patient on whether it is clinically appropriate and practical to accept you.

 

If accepted, you will attend the practice and receive the full range of services provided as normal at the surgery. If you have an urgent care need and require a home visit, you will need to call NHS 111 to arrange this.
